Adhering to public health guidelines for staff and student safety has been, and will continue to be, Union ESD’s priority throughout this pandemic and determines how we make decisions about teaching and learning.
Please participate in the daily at home health screening and reporting and familiarize yourself with the reporting procedures for a positive COVID-19 test.
Employee Health-Screening Form
The following is provided for an employee’s personal use to document his or her symptoms and recovery. This page should not be provided to the employer but kept for the employee’s personal records.
If you have a temperature of 100.4 degrees Fahrenheit, please remain home, and contact your health care provider.
Determination for release from isolation will be determined by following the latest guidance from Maricopa County Public Health and CDC.
Please note: Guidelines change regularly, return to work will be determined by the most up to date CDC guidelines. Given the statewide push for testing, we encourage staff to get tested, if they are exhibiting COVID-19 like symptoms and remain home.
Students and Staff may return earlier if a doctor confirms the cause of your fever or other symptoms is not COVID-19 and provides a written release to return. Staff must sign a COVID-19 Release to Return to Work form with the Human Resources Department prior to returning to the site after quarantine.
